The comprehensive form is below in PDF format for your convenience. This version contains the general information needed for Hutch Camp, the t-shirt order form, and the waiver forms. Players must bring the completed waiver forms with them for the first Saturday that they attend or they will not be able to participate. The waiver forms are also available separately.
